Caspofungin is an echinocandin antifungal that inhibits 1,3-beta-D-glucan synthase, the enzyme fungal cells use to build their cell wall, an action mammalian cells do not share. It is given only by intravenous infusion over roughly one hour, never as an oral dose or an IV bolus. The SmPC's licensed indications are invasive candidiasis in adults and children, invasive aspergillosis in patients refractory to or intolerant of other antifungal therapy, and empirical therapy for presumed fungal infection in febrile neutropenic patients. Standard adult dosing is a single 70mg loading dose on day one, then 50mg once daily; hepatic-impairment and several drug-interaction adjustments apply. It is supplied as 50mg and 70mg vials of powder for concentrate for solution for infusion, reconstituted with water for injection before dilution into saline or lactated Ringer's for the infusion bag.

What the dispensing pharmacist checks on caspofungin.
Caspofungin dosing is not one-size-fits-all: standard adult therapy is a 70mg loading dose on day one, then 50mg once daily, but moderate hepatic impairment (Child-Pugh 7-9) drops the maintenance dose to 35mg daily after the same loading dose, and severe impairment has no defined regimen. Reconstitute with water for injection, never a diluent containing glucose, since caspofungin is not stable in glucose-containing diluents, then dilute into saline or lactated Ringer's; the reconstituted concentrate holds 24 hours at or below 25°C, and the diluted infusion 48 hours refrigerated or 24 hours at room temperature. Rifampicin and other enzyme inducers push the maintenance dose to 70mg daily. Ciclosporin raises caspofungin exposure by around 35%, so review liver enzymes. Tacrolimus troughs fall by around 26% on caspofungin, so tacrolimus needs its own dose review. Unopened vials stay refrigerated at 2 to 8°C throughout transit.

What cold chain does caspofungin need through dispatch?

Does the caspofungin dose change for our hepatic-impairment patients?
Yes. No adjustment is needed for mild hepatic impairment (Child-Pugh 5-6). For moderate impairment (Child-Pugh 7-9), the SmPC recommends a 70mg loading dose on day one followed by 35mg daily. There is no clinical experience in severe hepatic impairment, so the SmPC advises caution rather than a defined dose.
Which co-medications does our pharmacy need to flag against caspofungin?
Rifampicin and other enzyme inducers raise the daily dose to 70mg after the loading dose. Ciclosporin increases caspofungin exposure by around 35%, so liver enzymes need closer monitoring. Tacrolimus trough levels fall by around 26% on caspofungin, so tacrolimus dosing needs its own adjustment. Flag all three at reconciliation.
