Prazosin is a selective, competitive alpha-1 adrenoceptor antagonist. It blocks postsynaptic alpha-1 receptors on vascular smooth muscle, relaxing both arterioles and veins and lowering peripheral vascular resistance, without the reflex tachycardia that older non-selective agents in the same broad antihypertensive family can trigger. It is dosed orally, not injected, and needs no reconstitution. The FDA label's indication is treatment of hypertension, to lower blood pressure and so reduce the risk of fatal and non-fatal cardiovascular events, used alone or combined with a diuretic or beta-blocker. Prazosin carries a first-dose effect: a small proportion of patients respond with marked postural hypotension, dizziness or syncope within 30 to 90 minutes of the first capsule, so both the FDA and UK labelling call for a low starting dose (1 mg or 500 micrograms two or three times daily), given at bedtime, before any titration. Maintenance doses build slowly to a usual ceiling of 15 to 20 mg daily in divided doses, with some patients titrated to 40 mg. Because a treatment gap of even a few days can reset that first-dose risk, medicines reconciliation should flag any recent non-adherence before a dose is resumed at its prior level rather than restarted low.
