Dulaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist, a fusion protein combining a modified GLP-1 peptide with a human IgG4 Fc fragment. It slows gastric emptying, stimulates glucose-dependent insulin secretion and suppresses glucagon release.
Trulicity is FDA-licensed as an adjunct to diet and exercise to improve glycaemic control in adults and paediatric patients aged 10 years and older with type 2 diabetes mellitus, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors. Other licensed indications may apply per destination-country labelling.

What the dispensing pharmacist checks on dulaglutide.
Dosing starts at 0.75 mg once weekly by subcutaneous injection. After at least 4 weeks, the dose may step up to 1.5 mg once weekly for additional glycaemic control, and if further control is needed it can be increased in 1.5 mg increments (to 3 mg, then 4.5 mg) after at least 4 weeks on the current dose, up to a maximum of 4.5 mg once weekly. This escalation pathway above 1.5 mg applies to adult patients only: for paediatric patients aged 10 years and older, the maximum recommended dose is 1.5 mg once weekly. The pen can be injected at any time of day, with or without food, and the injection day can be changed if the last dose was given at least 3 days before.
Missed-dose rule: if at least 3 days (72 hours) remain until the next scheduled dose, administer the missed dose as soon as possible. If fewer than 3 days remain, skip the missed dose and resume on the regular schedule. Do not double up.
Cold chain is the operational risk with this product. Pens are stored at 2-8C in the manufacturer's carton, protected from light, from dispatch through to the pharmacy fridge. Once removed from refrigeration, a pen may be kept below 30C for up to 14 days before it must be discarded; this room-temperature allowance is cumulative across however many times the pen leaves the fridge, and it is intended for the patient's use window, not for distributor-level storage or transit. Importers should plan validated 2-8C transport for every leg of the journey and treat the 14-day room-temperature window as a patient-facing buffer only.
Injection sites rotate between the abdomen, thigh and upper arm, with a different site used each week even within the same body area. No biosimilar or generic dulaglutide is licensed anywhere at present, so substitution is not a consideration for this product; supply continuity depends entirely on the authorised distribution chain.
