It inhibits both cyclo-oxygenase-1 and cyclo-oxygenase-2, reducing prostaglandin synthesis to relieve pain and inflammation, with a COX-2 preference that is more pronounced in its topical gel form than in the oral tablet. It is dosed orally, rectally or by intramuscular injection, or applied topically; no reconstitution is needed for any presentation. Diclofenac's class-wide NSAID warning is the dispensing point that matters most: the FDA label carries a boxed warning for cardiovascular thrombotic events and for gastrointestinal bleeding, ulceration and perforation, and the EU SmPC carries an equivalent caution, so the lowest effective dose for the shortest duration is the working rule at every strength. Diclofenac sodium delayed-release tablets are contraindicated around coronary artery bypass graft surgery, and diclofenac potassium immediate-release tablets are contraindicated in the third trimester of pregnancy. Total daily dose should not exceed 150 mg across oral, rectal and injectable routes combined if more than one presentation is co-prescribed, since patients switching from suppository to tablet mid-course are an easy double-dosing error to miss on medicines reconciliation. The intramuscular injection is for deep gluteal administration only, never intravenous push, and is typically limited to one to two days before stepping down to oral or rectal dosing. Diclofenac raises lithium, digoxin and methotrexate plasma levels and reduces the antihypertensive effect of ACE inhibitors and diuretics, so a concurrent-medicines check belongs on every dispensing label.
