Granisetron is a selective 5-HT3 (serotonin) receptor antagonist. It blocks serotonin binding at vagal afferent and central chemoreceptor-trigger-zone sites, which interrupts the reflex that drives emetogenic chemotherapy- and radiotherapy-induced nausea and vomiting, without the dopamine-blocking side effects seen in older antiemetic classes. Kytril's FDA-labelled indications are prevention of nausea and vomiting from initial and repeat courses of emetogenic cancer therapy, including high-dose cisplatin, and prevention and treatment of postoperative nausea and vomiting in adults; the UK/EU SmPC for Kytril tablets extends to prevention of delayed nausea and vomiting after chemotherapy and radiotherapy. It is not indicated for chronic or unrelated nausea.

What the dispensing pharmacist checks on granisetron.
Chemotherapy-prophylaxis dosing is weight-based, not fixed: 10 mcg/kg IV within 30 minutes before the start of chemotherapy, the same regimen used in paediatric patients aged 2 to 16. Serotonin syndrome has been reported with 5-HT3 antagonists, particularly alongside other serotonergic medicines such as SSRIs, SNRIs and tramadol, so medicines reconciliation should check for these before dispensing. Granisetron can prolong the QT interval and warrants caution in patients with pre-existing arrhythmia or cardiac-conduction disorders. The Sancuso patch needs its own counselling: applied to the upper outer arm 24 to 48 hours before chemotherapy, worn for up to seven days, kept away from heat sources and covered from direct sunlight during wear and for ten days after removal.
