Netilmicin is a semi-synthetic aminoglycoside antibiotic, derived from sisomicin, that inhibits bacterial protein synthesis by binding the 30S ribosomal subunit. It is bactericidal against a range of Gram-negative organisms including E. coli, Klebsiella, Enterobacter, Citrobacter and Proteus species, and against some Staphylococcus species, with a spectrum built specifically to cover strains that have become resistant to gentamicin. As Netromycin, the originator injection is indicated for septicaemia, lower respiratory tract infections, complicated and uncomplicated urinary tract infections, peritonitis, endometritis and bone or soft-tissue infections caused by susceptible organisms, per the Canadian CPS/Schering monograph. A separate licensed presentation, netilmicin sulfate 3 mg/mL combined with dexamethasone 1 mg/mL eye drops, is authorised in the UK for inflammatory anterior-segment eye conditions where bacterial infection or infection risk is present, including post-cataract surgery cover. Supplied as netilmicin sulfate injection in 50 mg/mL (2 mL vial) and 100 mg/mL (2 mL and 20 mL pharmacy bulk vial) strengths, and as a 3 mg/mL netilmicin/1 mg/mL dexamethasone ophthalmic solution. The injection needs no reconstitution; it is drawn and given intramuscularly or by slow intravenous infusion.

What the dispensing pharmacist checks on netilmicin.
Adult dosing for uncomplicated urinary infection is 4 mg/kg/day in two divided doses 12-hourly; for systemic infection, 4-6 mg/kg/day in two to three divided doses; serious infection by the IV route can run to 7.5 mg/kg/day. Neonatal and paediatric dosing runs higher on a mg/kg basis (children 6-7.5 mg/kg/day in three 8-hourly doses; neonates 6-9 mg/kg/day, age-banded), which matters because most of our current order flow sits in neonatal and burns-unit tenders. In renal impairment, the 8-hourly interval is stretched by multiplying serum creatinine by a factor of eight, and serum-level monitoring is recommended wherever the lab can turn it around. Do not co-infuse with beta-lactams in the same line. Storage is 2-30C, protect from freezing: a controlled room-temperature range wide enough that this does not need validated cold-chain freight, which simplifies last-mile handling into facilities without reliable refrigeration.

Netromycin's systemic injection is not currently active on FDA or DailyMed labelling and has been discontinued from several regulated markets on commercial rather than safety grounds, so buyers in those geographies typically source it under a named-patient or Specials-type import route rather than a standard formulary listing; M Care structures the paperwork accordingly. The UK licensing picture is narrower still: the MHRA-registered netilmicin product on the market today is the netilmicin/dexamethasone combination eye drop, not a systemic injection, so a UK buyer asking for injectable netilmicin is asking for an import, not a stocked NHS line. Netilmicin itself does not sit on the WHO core Model List of Essential Medicines, but the WHO Expert Committee has named netilmicin ocular preparations, alongside amikacin, kanamycin and tobramycin, as an accepted square-box therapeutic alternative to gentamicin eye drops for infectious blepharitis and conjunctivitis, which is the closest the molecule gets to an EML listing.
