Valaciclovir is the L-valyl ester prodrug of aciclovir, a nucleoside analogue that, once phosphorylated by viral thymidine kinase inside an infected cell, inhibits herpes DNA polymerase and halts viral replication. After oral dosing it is rapidly and almost completely converted to aciclovir by intestinal and hepatic enzymes, which is why it is dosed as a tablet rather than injected, and why fewer daily doses achieve aciclovir exposure that oral aciclovir itself cannot match. Dosing is indication-led, not one regimen: cold sores take 2 g twice in a single day twelve hours apart; recurrent genital herpes is 500 mg twice daily for three days while an initial episode runs 1 g twice daily for up to ten days; suppressive therapy drops to 500 mg once daily in immunocompetent adults; shingles is 1 g three times daily for seven days, extended in immunocompromised patients until lesions crust; and UK/EU-licensed CMV prophylaxis after transplant is a much higher 2 g four times daily for around ninety days. Aciclovir, the active metabolite, is cleared renally, so every regimen needs dose or interval adjustment once creatinine clearance falls below 50 mL/min, applying the manufacturer's renal-adjustment table at dispensing rather than a flat reduction. Thrombotic thrombocytopenic purpura and haemolytic uraemic syndrome, in some cases fatal, have been reported in patients with advanced HIV-1 disease and in transplant recipients on high-dose regimens, so any unexplained bruising, confusion or falling platelet count during high-dose therapy warrants immediate discontinuation.
