Candesartan cilexetil is an angiotensin II receptor blocker (ARB). The US Atacand label (ANI Pharmaceuticals, Inc., distributor; manufactured by AstraZeneca AB; revision 07/25) lists two indications: hypertension in adults and in children 1 to under 17 years of age, and heart failure (NYHA class II to IV) in adults with left ventricular systolic dysfunction (ejection fraction 40% or less), to reduce cardiovascular death and heart failure hospitalisation. The US tablets are 4, 8, 16 and 32 mg. The UK emc SmPC we read for a candesartan cilexetil tablet (Milpharm) lists primary hypertension in adults, hypertension in children and adolescents aged 6 to under 18 years, and adult heart failure with ejection fraction 40% or less when ACE inhibitors are not tolerated or as add-on therapy. In the UK, the emc also lists the Amias brand of candesartan cilexetil tablets from Neon Healthcare Ltd. The Irish regulator (HPRA) lists Atacand 8 mg tablets as authorised, held by Cheplapharm Arzneimittel GmbH, with an authorisation date of 29 April 2002. Candesartan is not labelled for migraine.

What the dispensing pharmacist checks on candesartan.
Dose by indication, not by a single number. The US label starts adult hypertension at 16 mg once daily for patients who are not volume depleted, given once or twice daily with total daily doses of 8 to 32 mg, while the UK SmPC we read starts at 8 mg once daily, allows 4 mg in patients at risk of hypotension or with renal impairment, and recommends 4 mg in mild to moderate hepatic impairment. In heart failure both start at 4 mg once daily and double at intervals of about two weeks (UK: at least two weeks) toward 32 mg once daily, as tolerated. Paediatric hypertension has its own age bands and weight-based limits, and the US and UK age ranges differ (1 to under 17 years in the US, 6 to under 18 in the UK SmPC), so check the label of the destination. The US label carries a boxed warning on fetal toxicity: when pregnancy is detected, discontinue Atacand as soon as possible. The UK SmPC contraindicates ARBs in the second and third trimesters and does not recommend them in the first. The UK SmPC states the tablet can be divided into equal doses.

Candesartan is a room-temperature product. The US Atacand label gives 20 to 25 °C with excursions to 15 to 30 °C and asks for the container to be kept tightly closed. The UK emc SmPC for a candesartan cilexetil 16 mg tablet gives storage below 30 °C and a 2-year shelf life. The US label states that Atacand is a registered trademark of the AstraZeneca group of companies, licensed to ANI Pharmaceuticals, Inc., and that the tablets are manufactured by AstraZeneca AB under licence from Takeda Pharmaceutical Company, Ltd. In the UK, the emc lists Amias (Neon Healthcare Ltd) alongside candesartan cilexetil tablets sold under the ingredient name by holders such as Milpharm, Krka and Sandoz. Is candesartan used for migraine?
Migraine is not a labelled indication for candesartan. The US Atacand label lists hypertension and heart failure, and the UK SmPC we read lists primary hypertension, paediatric hypertension and heart failure. M Care Exports does not promote any use outside the labelled indications.
Is candesartan with hydrochlorothiazide the same product?
No, the combination is a separate product. In the US, Atacand HCT (candesartan cilexetil and hydrochlorothiazide) is listed on DailyMed and Drugs@FDA under NDA021093, in 16/12.5, 32/12.5 and 32/25 mg strengths, and generic combinations are approved. The US label for Atacand HCT states it is not indicated for initial therapy.
