What the dispensing pharmacist checks on haloperidol.
Keep the three presentations apart on the requisition, because they are not interchangeable. The Haldol Decanoate label gives the first dose as 10 to 15 times the previous daily oral dose for patients under 65 stabilised on up to 10 mg a day with normal hepatic function, with a maximum initial dose of 100 mg; a larger calculated dose is split, 100 mg first and the remainder 3 to 7 days later. It is given by deep intramuscular injection by a healthcare professional every 4 weeks and never intravenously. The UK 5 mg/mL injection SmPC recommends intramuscular use only and says intravenous use needs continuous ECG monitoring, because QTc prolongation, ventricular arrhythmias and sudden death are reported, with risk appearing to rise with high doses, high plasma concentrations, predisposed patients and parenteral, particularly intravenous, use. It recommends a baseline ECG before intramuscular dosing, correction of hypokalaemia and hypomagnesaemia before treatment, and stopping if QTc exceeds 500 ms. Its contraindications include Parkinson's disease, dementia with Lewy bodies and concomitant medicines that prolong the QT interval. Storage is easy: the US decanoate label says 20 to 25 °C, protect from light, do not refrigerate or freeze, and the US tablet label says 20 to 25 °C in a tight, light-resistant container.

The US generic tablet label says 20 to 25 °C in a tight, light-resistant container. The Teva UK 5 mg tablet SmPC says store below 25 °C and protect from light, with a 24-month shelf life.
