Lidocaine is an amide-type local anaesthetic that blocks voltage-gated sodium channels in nerve membranes, stopping the initiation and conduction of the nerve impulse that carries pain signals. The same channel-blocking action, at lower systemic doses, suppresses ectopic ventricular activity, which is why the same molecule appears twice on a hospital formulary under two different uses. The originator's US labelling covers infiltration, peripheral nerve block and intravenous regional anaesthesia; the UK SmPC additionally covers epidural and caudal use and adds treatment of ventricular extrasystoles and ventricular tachycardia at the 2% strength. It is dosed by weight, with a standard adult ceiling of 4.5 mg/kg or 300 mg for infiltration and nerve block without epinephrine, rising to 7 mg/kg or 500 mg with epinephrine.
